Grantmaking Program Area Descriptions:
Access to Health Care: We support programs that focus on providing access to quality health care for at-risk and underserved populations. Applications accepted in the following areas: Community health centers and clinics and mobile health clinics.
Wellness & Prevention: We support programs that build healthy habits and that are designed to help people achieve their best possible health outcomes. These programs can be in a community setting or in a public school. Applications accepted in the following areas: Wellness programs that focus on building healthy habits including but not limited to chronic disease management, nutrition and exercise and health screening; Wellness programs that take place in a public school setting; Inclusive programs targeted at supporting wellness initiatives for children with disabilities such as physical activity and play.
Health Education & Awareness: Optimal community health begins with education and awareness. We support porgrams that raise awareness of important health issues, focus on chronic disease management and offer training and other tools that our communities need to help them on their path to better health. Applications accepted in the following areas: Health educations materials; Health seminars and symposiums; Health screenings; Early intervention, screenings and programs.
