Main Areas of Responsibility:
Provides intensive case management and services coordination, focused on housing stability.
Assists clients with securing appropriate resources for mental health services and treatment and substance use management
Assists clients in accessing supportive services and applying for public benefits
Assists clients with money management and other independent living skills training and assistance
Connects clients to education and vocational training as well as career/job counseling and development
Additional Areas of Responsibility:
Maintains accurate records of services provided, completed referrals, and clients' files
Communicates clients' progress by participating in weekly interdisciplinary meetings and evaluations
Prepares housing disposition reports to accurately capture guests’ exit plan and post-discharge requirements.
Provides additional resources, supports and services to clients that support the Individualized Development Plan.
Monitors parents’ protective capacities and models appropriate parenting
Determines clients' needs by conducting Family Needs Assessments and ISPs.
Responsible for keeping timely case notes in FAMCare C/M Software.
Attends Family Centered Meetings (FCM) and court hearings as needed.
Responsible for connecting clients to other affordable housing opportunities to ensure a smooth transition to stable housing.
