Con Alma Health Foundation is soliciting nominees for our eighth annual Hero of Health award given to individuals who make a significant difference in the health of their community. To nominate someone, send us a brief description of why you believe the person is a hero. Heroes can come from any field, including health-care providers, policy makers, advocates, clinicians, administrators and volunteers who give of their time to address unmet health needs in New Mexico.
We would love to hear about New Mexicans who are making sustainable changes to improve people’s access to healthy food and physical activity in their communities—actions that can help achieve health equity, when all people have an equal chance at living healthy lives regardless of their zip code, income or ethnicity.

Nominate a Hero of Health by sending us the reason you are honoring this person, with a photo, if you have one. A Selection Committee will choose one 2016 Hero of Health and a $1,000 donation will be given to a New Mexico nonprofit of the honoree’s choosing.
All nominees will be recognized as part of our Annual Grantee Recognition Reception on November 17, at the Inn and Spa at Loretto in Santa Fe.
