Main Areas of Responsibility:
Provide onsite case management and supportive housing services to tenants of Hope Village. The Supportive Housing Case Manager will be part of a team that will be providing onsite wrap-around services to assist tenants in being successful in housing. These services include building rapport and trusted relationships, conducting home visits, coordinating and ensuring that tenant is engaged in and receiving all necessary support services, and attending and participating in all team meetings
Additional Areas of Responsibility:
Conduct home visits; Complete all necessary documentation within the expected timeframe; Assist clients in understanding their lease and community rules and expectations; Work with property manager and other support staff to address challenges and develop creative and appropriate solutions; Assist in coordination of additional support services and ensure tenant is receiving all necessary services; Create housing success plans and track progress towards goals;
