Rapid Rehousing Case Manager

Job Description
Type of Job: 
Full-Time
Main Areas of Responsibility: 

• Conduct participant screenings and intakes.
• Support housing placement.
• Create a Housing Sustainability Plan for each participant.
• Monitor client progress.
• Provide crisis intervention as necessary.
• Provide information and referrals to community resources.

Additional Areas of Responsibility: 

Required skills:
• Must have excellent computer skills with a proficiency in Microsoft Word and Excel
• Effective written and oral communication skills
• Ability to relate to individuals and families experiencing homelessness
• First Aid and CPR certifications
• Valid Driver’s License and reliable transportation
• Knowledge of community resources
• Previous housing management/case management experience desired

Other Information/Requirements: 

Education:
• Bachelor’s degree in counseling, social work, social sciences, or related studies preferred or
• Minimum of two years of case management or commensurate experience with transferable skills
Scheduling:
• Standard Schedule: Monday – Friday, 40 hours a week
• Flexibility to conduct meetings evenings or weekends, as needed
