Main Areas of Responsibility: 

This position is responsible for the case management functions related to the delivery of caregiver educational classes and support groups across NM, relating to Alzheimer's Disease and other Dementias.

Additional Areas of Responsibility: 

Additional Areas of Responsibility include training, support, linkage and follow-up to volunteer and staff providing services, provide care consultations to family caregivers and be comfortable and proficient with public presentations.

Other Information/Requirements: 

The candidate should possess ability to work with staff, volunteers and diverse communities, have excellent verbal and written communication skills, organizational skills, professional customer service skills. BA degree in health or human services related or AA degree and three (3) years of experience performing case management duties in a health or human services field, or a minimum of five (5) years experience case management in health or human services field.
