Main Areas of Responsibility: 

This position is responsible for the case management functions related to the delivery of caregiver educational classes and support groups across NM; to include, coordination, training, support, linkage and follow-up. Ensure Savvy Caregiver classes and support groups are coordinated, scheduled and provided in safe, timely and effective manner.

Additional Areas of Responsibility: 

Ensure sufficient number of Savvy Caregiver educators and provide and coordinate training. Maintain comprehensive records and current data entry. Provide and instruct caregiver education classes in the Greater Albuquerque Metro area.

Other Information/Requirements: 

BA degree in health or human services related field; or AA degree and three (3) years of experience performing case management duties in a health or human services field, or; Experience as a case manager with a minimum of five (5) years experience in working with the target population.
