Main Areas of Responsibility: 

Casa de Salud has an opening for a full-time Pathways Navigator to continue this critical case management work. Casa’s Pathways Navigators have historically had a strong relationship and support network among the Pathways Navigators at various community-based organizations around the county.

Additional Areas of Responsibility: 

The Pathways Navigator would join a growing team at Casa de Salud, and would work integrally with another full-time case manager at the organization – the Intensive Case Manager, as well as with a wide range of integrative and primary care clinicians and staff. The Navigator would also have the opportunity to participate in numerous leadership development and professional development opportunities.

Other Information/Requirements: 

For this role, Spanish proficiency is required, given a significant number of community members who access services at Casa de Salud or who live in the South Valley and in surrounding areas, speak Spanish.
