Main Areas of Responsibility:
Under indirect supervision, this position is responsible for carrying a caseload of clients, who are homeless, previously incarcerated, history of prostitution, have a co-occurring mental health diagnosis. Provides intensive case management services including crisis intervention and develops treatment plan in conjunction with other Community Health Workers.
Additional Areas of Responsibility:
Refers clients to other supports services as needed. Monitors condition of clients to evaluate success and adapts treatment program as needed.
