Part Time Case Manager

Job Description
Main Areas of Responsibility: 

Provides case management services including crisis intervention and emergency services, ongoing psychosocial assessment, develops treatment plan in conjunction with appropriate staff members and client including discharge planning. Provides support and encouragement for clients in fulfilling treatment plans and achieving goals.

Additional Areas of Responsibility: 

This position is responsible for carrying a caseload of current and possibly aftercare clients, who are homeless, previously incarcerated and have a co-occurring mental health diagnosis with a drug/alcohol dependency.Has an understanding of the principles of trauma treatment and trauma informed care, harm reduction, and strengths based substance abuse and mental health recovery.

Other Information/Requirements: 

Ability to work effectively in a rapidly changing, multi-task, crisis oriented environment. Ability to work independently and as part of the team to ensure that the highest quality and most relevant services are provided to clients. In collaboration with the Clinical Director and other designated staff prepare for vacant housing units by communicating with referring agencies and applicants about status on the waitlist, notification of interviews, and outcome of interviews and acceptance to the program.
