Lead Case Manager

Job Description
Type of Job: 
Full-Time
Main Areas of Responsibility: 

- Facilitates client access to community resources
-Coordinates and monitors services, including comprehensive tracking of client activities in relation to service plan.
-Assists clients in developing goals and areas of need.
-Assesses client's progress, evaluates effectiveness of service plan and progress made by client.
-Responsible for direct supervision of all case managers.
-Provides guidance and support to Case Managers in a group setting as well as individually.

Additional Areas of Responsibility: 

-Implement and enforce CYFD domestic violence procedures, policies, and guidelines as they pertain to case managers.
-Implement and enforce policies, procedures, and guidelines of other intervention programs including but not limited to Pathways, MFA, and OVW.

Other Information/Requirements: 

Bachelor's degree and minimum 2 years of experience that is directly related to the duties and responsibilities specified or high school diploma or equivalency with five years work experience, at least two of which are directly related.
