FT Case Manager

Job Description
Main Areas of Responsibility: 

This position is responsible for carrying a caseload of current and possibly aftercare clients, who are homeless, previously incarcerated and have a co-occurring mental health diagnosis with a drug/alcohol dependency.

Additional Areas of Responsibility: 

Provides case management services including crisis intervention and emergency services, ongoing psychosocial assessment, develops treatment plan in conjunction with appropriate staff members and client including discharge planning. Provides support and encouragement for clients in fulfilling treatment plans and achieving goals.

Other Information/Requirements: 

Ability to work effectively in a rapidly changing, multi-task, crisis oriented environment. Ability to work independently and as part of the team to ensure that the highest quality and most relevant services are provided to clients.
