Main Areas of Responsibility: 

The purpose of this position is to provide peer services and supportive housing services to formerly homeless individuals and families. This includes, but is not limited to, monthly home visits, planning, budgeting, and referrals to other services.

Additional Areas of Responsibility: 

This position will also help document and maintain daily progress notes of client contacts and services provided; assist clients in finding apartments; and act as a resource for programs in specialty areas and by networking with appropriate agencies, shelters, meal sites, etc., as needed.

Other Information/Requirements: 

The candidate must have current certification as a CPSW; one year's experience working with adult severe mental illness (SMI) population; excellent interpersonal skills; high proficiency with Microsoft Office; effective time management skills; and ability to work independently displaying good judgment and problem-solving skills. Experience working with people who do not have housing is preferred.
