Case Manager

Job Description
Main Areas of Responsibility: 

Provides case management services including crisis intervention and emergency services, ongoing psychosocial assessment, develops treatment plan in conjunction with appropriate staff members and client including discharge planning. Provides support and encouragement for clients in fulfilling treatment plans and achieving goals.

Additional Areas of Responsibility: 

Bachelor’s Degree in social work or a combination of relevant education, training, and experience totaling 4 years. A Master’s Degree in social welfare strongly preferred.

Other Information/Requirements: 

Must have an understanding of the principles of trauma treatment and trauma informed care, harm reduction, and strengths based substance abuse and mental health recovery. Must have strong attention to detail, and be well organized. Must demonstrate a commitment to treating others with respect.
