Main Areas of Responsibility:
This case manager will work specifically with families who are involved in child welfare and have been referred to the Keeping Families Together (KFT) program by CYFD. The case manager will provide intensive case management services to a maximum of 20 families, focused on housing stability and child safety. Monitors parents' protective capacities and models appropriate parenting.
Additional Areas of Responsibility:
Determines clients' needs by conducting Family Needs Assessments and goal plans. Assists clients with securing appropriate resources for mental health services and treatment and substance use management. Connects clients to education and vocational training as well as career/job counseling and development. Communicates clients' progress by participating in weekly interdisciplinary meetings and evaluations. Attends Family Centered Meetings (FCM) and court hearings as needed.
