Case Manager

Job Description
Type of Job: 
Full-Time
Main Areas of Responsibility: 

-Facilitates client access to community resources
-Coordinates and monitors services
-Documents all client encounters and contacts made on behalf of clients
-Completes and submits events on Clienttrax database and other documentation as appropriate
-Maintains comprehensive client cases
-Assists clients in developing goals in areas of need and assists in developing service plans

Additional Areas of Responsibility: 

-Identifies and provides emergency crisis services as necessary
-Assesses client's progress, evaluates effectiveness of service plan and progress made by clients, and participates in client treatment planning and case review with other staff members.
-Maintains current information regarding programs and services available to clients in the community.
-Reviews intakes, client cases, and monitor if service goals are met.
-Maintain client list.

Other Information/Requirements: 

Bachelor's degree and minimum 1 year of experience that is directly related to the duties and responsibilities specified, or high school diploma or equivalency with five years work experience, at least two of which are directly related. (EOE)

Individual will be subject to a CYFD background check.
