Main Areas of Responsibility:
-Facilitates client access to community resources
-Coordinates and monitors services
-Documents all client encounters and contacts made on behalf of clients
-Completes and submits events on Clienttrax database and other documentation as appropriate
-Maintains comprehensive client cases
-Assists clients in developing goals in areas of need and assists in developing service plans
Additional Areas of Responsibility:
-Identifies and provides emergency crisis services as necessary
-Assesses client's progress, evaluates effectiveness of service plan and progress made by clients, and participates in client treatment planning and case review with other staff members.
-Maintains current information regarding programs and services available to clients in the community.
-Reviews intakes, client cases, and monitor if service goals are met.
-Maintain client list.
