Main Areas of Responsibility:
This position is responsible for carrying a caseload of current and possibly aftercare clients, who are homeless, previously incarcerated and have a co-occurring mental health diagnosis with a drug/alcohol dependency. Provides case management services including crisis intervention and emergency services, ongoing psychosocial assessment, develops treatment plan in conjunction with appropriate staff members and client including discharge planning.
Additional Areas of Responsibility:
Provides support and encouragement for clients in fulfilling treatment plans and achieving goals. Has an understanding of the principles of trauma treatment and trauma informed care, harm reduction, and strengths based substance abuse and mental health recovery.
