Care Coordinator

Main Areas of Responsibility: 

NLH provides supportive housing for the most vulnerable in the community. The Care Coordinator completes resident assessments, and identifies community resources tailored to individual needs. Monitors support services and ensures client documentation is in compliance with grants and numbers served, assists clients with financial, ;literacy, computer training and social activities.

Additional Areas of Responsibility: 

Promotes tenant self sufficiency, crisis intervention, collaborates with agencies assisting tenants, works with Volunteer Coordinator, Site Managers, and Maintenance staff. Participates in staffing meetings on weekly basis. Promotes continuity of services and preserves client privacy.

Other Information/Requirements: 

High School Diploma/GED. Any degrees in social services
2 year’s minimum experience working with at risk population
Knowledge of culture and values in the local community
Familiarity with the resources available within the community
Is a self-starter that requires minimum supervision
Bilingual is a plus
