Main Areas of Responsibility:
The YWCA is seeking a full-time Case Manager for our transitional living center for homeless women veterans & their dependent children. This position supports residents in their goal to achieve self-sufficiency & is part of an interdisciplinary support team including the program manager & program director. The team works together to assist residents with implementing their independent development plan goals in order to move into permanent housing with the skills needed to be successful.
Additional Areas of Responsibility:
The Case Manager is responsible for day-to-day case mgmt & supporting residents. The successful candidate will have knowledge of veteran issues & an understanding of medical terminology, knowledge of homeless populations & transitional supportive care, the ability to effectively present information in one-on-one & group situations to internal & external stakeholders, excellent people skills, & ability to effectively communicate any concerns related to the residents to the Program Director.
