Main Areas of Responsibility:
Responsible for recruiting, training and placement of volunteers for in home care. Work with the City and County of Santa Fe on referred residents needing help with health navigation, which includes assessing referred clients' physical and mental wellness, needs, preferences and abilities, identifying and implementing appropriate interventions, and developing collaborative service plans and monitoring mutual progress.
Additional Areas of Responsibility:
Working closely with the Operations Manager and Executive Director to ensure clear communication among different aspects of the agency to fulfill our mission; attending weekly staff meetings; providing documentation of clients navigated and volunteer placement on a monthly basis; working with the Director to ensure that all grant reporting is accurate and submitted in a timely manner; assisting clients in acquiring durable medical equipment from our loan program; and other duties as assigned.
