South Valley Community Health Program Director

Job Description
Main Areas of Responsibility: 

The primary objective of this position is to expand the Pathways for a Healthier Bernalillo County model to a model that will create significant change. Goals: 1. Increase the number of Community Navigators; 2. Improve coordination, communication & capacity efforts in the South Valley; 3. Improve health outcomes (individual & policy) by collectively addressing systemic issues through an advocacy structure that will mitigate structural barriers to access.
Compensation is $40k - $60k DOE

Additional Areas of Responsibility: 

The EleValle Director, under the direction of the EleValle Leadership, will support community health navigators as they provide services, support & referrals to marginalized populations (such as immigrant, returning citizens, elders, veterans, etc.); Identify new & strengthening existing programs & services; assessment & evaluation activities for grants; Cultivate relationships & improving community awareness, resource sharing & technical support to improve the health of South Valley families.

Other Information/Requirements: 

Goals: To provide program development, coordination, management, & report writing to address issues identified through Pathways, to create processes to analyze & address downstream & upstream health barriers identified by navigators, including engaging the community in identifying key issues, to build effective policy & advocacy initiatives to address priority issues, & to develop & leverage funding & resources for effective programs & services that reduce/eliminate gaps to access within the health system.
