Service Coordinators

Job Description
Main Areas of Responsibility: 

Provide ongoing assistance and support to residents in supportive housing communities.
Will provide information and supportive access to local services and resources. Required to conduct one monthly home visit to residents minimally.
Will be responsible for keeping all required documentation in order.

Additional Areas of Responsibility: 

This position is scheduled to work 36 hours week and is eligible for full-time benefits.

Other Information/Requirements: 

BA in a Human Services field or equivalent.
2 years experience working with underserved populations.
Must have working knowledge of mental health and substance use issues.
Experience with electronic medical record, HMIS and bilingual preferred.
