Service Coordinator for Transitional Living Program

Job Description
Main Areas of Responsibility: 

Provide case management services from screening and intake to discharge including: completing client assessments; developing self-sufficiency plans, implementing, coordinating and reviewing; and providing discharge and after-care planning and implementation for an assigned case load. Provide life skills instruction on an individual basis, following established self-sufficiency plans and conduct daily life skills groups on a variety of topics.

Other Information/Requirements: 

Two to Four years of relevant experience required depending on educational background. Masters Degree (preferred). See website for full listing of requirements. $14.28 per hour with medical, dental, and paid time off.
