Service Coordinator

Main Areas of Responsibility: 

The Service Coordinator uses a person-centered approach to offer supportive services to persons emerging from homelessness in scattered-site, supportive housing settings. Services include initial assessments, developing Individual Service Plans (ISP), crisis intervention, increasing access to benefits/health care, community integration, and community development within the residential community.

Additional Areas of Responsibility: 

The Service Coordinator ought to be familiar with evidence-based practices such as: Housing First, harm reduction, motivational interviewing and trauma-informed care. The Service Coordinator works collaboratively with a range of community providers to ensure tenants are stabilizing in housing, improving the quality of their lives and integrating into the community.

Other Information/Requirements: 

People with lived experiences of homelessness, mental health recovery, substance use recovery, etc. are encouraged to apply.
BA/BS from an accredited college or university in Social Work or related Human Services field, or four years comparable work experience plus minimum two years’ experience working with individuals with serious mental illness and/or substance abuse issues, the homeless or other underserved populations.
