Full-Time
Main Areas of Responsibility: 

Provide ongoing assistance and support to residents of supportive housing communities to maintain their housing. Provide information and supportive access to local services and resources. Conduct monthly home visits. Responsible to keep all required documentation.

Other Information/Requirements: 

BA in a human services field or equivalent. Two years experience working with underserved populations. Must have working knowledge of mental health and substance use issues. New Mexico driver's license with clean driving record required. Experience with electronic health records, HMIS, and bilingual preferred.
