Screening & Referral Specialist

Job Description
Main Areas of Responsibility: 

Responsible for screening to determine eligibility for Behavioral Health Programs. Determine level of eligibility

Additional Areas of Responsibility: 

Provide information and referrals regarding other human service agencies; Act as contact person for referring agencies and track referrals; Gather and track PATH information and assist Program Managers in data reporting as required by PATH funding.

Other Information/Requirements: 

Bachelor’s degree in Social Work, Counseling or other related NM state human services field; At least 1 year of experience working in a human services setting; Experience working with adults with a chronic mental illness preferred; Experience working with adults with substance abuse problems preferred; Reliable; Basic computer skills; Bilingual preferred.
