Safe Home Care Coordinator

Full-Time
Main Areas of Responsibility: 

The Care Coordinator works to provide and organize necessary services and resources for youth 11-17 in the Safe Home setting. Identifying and arranging access to safe housing options, building bridges of communication between youth and their supports and between those supports and the Safe Home.

Additional Areas of Responsibility: 

• Support, engage, and empower young people in developing and meeting their goals
• Develop the Shelter Care plan within three days of youth entering the Safe Home, update every 30 days
• Act as the primary point of contact for coordinating services
• Be willing to adopt and utilize the core principles of the Safe Home in all interactions with youth and community partners: Positive Youth Development, The Nurtured Heart Approach and Trauma Informed Care

Other Information/Requirements: 

• Maintain a flexible schedule related to hours and days of work in order to meet the needs of youth and families
• Must understand the practice of good boundaries with youth

How to Apply: 

• Must be 21 yrs or older
• Bachelors in Social Work, Counseling, Psychology, or related field with 2 years’ experience working with youth
• Associates in related field with 3 years’ experience working with target population
• High School or equivalent with 4 years’ experience working with target population
