Safe Home Care Coordinator

Job Description
Type of Job: 
Full-Time
Main Areas of Responsibility: 

• Support, engage, and empower youth in developing and meeting goals.
• Develop a care plan within three days of youth entering Safe Home
• Maintain meticulous records of all youth, guardian, and resource contacts
• Complete monthly billing sheets to submit to CYFD
• Act as primary point of contact for coordinating services

Additional Areas of Responsibility: 

Be willing to adopt Safe Home core principals of Positive Youth Development, Nurtured Heart Approach, and Trauma Informed Care in all interactions with youth. Approach all interactions with youth and community partners with respect and collaboration. Maintain flexible schedule to meet needs of youth and families.

Other Information/Requirements: 

• Must be 21 or older
• Bachelor's degree in social work, counseling or psychology
or
• Bachelors degree in another field with two years experience working with youth
or
• Associates degree with three years experience working with youth
or
• High school or GED with four years experience working with youth
