Type of Job: 
Full-Time
Main Areas of Responsibility: 

Coordinate health care services for individuals on the DD Waiver per DDSD Standards.
Train staff on health care protocols and practices.
Write and update individualized health care plans.
Conduct nursing visits and attend doctor appointments.

Additional Areas of Responsibility: 

Participate in IDT and ISP meetings for client care coordination.
Maintain medication administration and treatment records.
Oversee LPNs and support the nursing team as an RN.

Other Information/Requirements: 

RN certification as a registered nurse (RN) in the state of New Mexico required.
Must maintain CPR, First Aid, and other trainings/certifications as required.
One year experience working with disabled individuals, including those with severe/profound disabilities, strongly preferred.
