Main Areas of Responsibility:
Under the direction of the HCH Director, the HCH RN is responsible to La Familia Medical Center (LFMC) patients for providing nursing care and individualized patient needs, in conformance with recognized nursing techniques, procedures, and established standards based on the scope of nursing. This includes long term coordination of care and services to homeless individuals, assisting them to overcome medical and social issues associated with the condition of homelessness.
Additional Areas of Responsibility:
Utilizes the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of patient care, as evidenced by patient/staff observation; maintains appropriate patient records for continuity of care, treatment planning, and patient tracking; prepares and submits all required patient and activity reports; and ensures proper patient demographic information is entered into computer systems at each visit if applicable, for homeless patients.
