Main Areas of Responsibility:
The Promotora/CHW will work directly with patients and a Nurse Practitioner providing mental health services assisting in appointment scheduling, referrals to community and internal resources.
Work directly with patients from diverse backgrounds who typically are underserved and lack access to healthcare.
Coach patients with a diverse array of health influences such as substance abuse, trauma, and economic hardship in effective management of their mental health and stressors.
Additional Areas of Responsibility:
Develop, manage, and facilitate support groups as needed (training will be provided)
Provide mental health screenings and perform vital signs assessment as needed under the supervision of licensed medical provider (training will be provided).
Collect data and write brief reports to meet evaluation needs of the program.
Participate in training, continuing education and certification programs.
