Main Areas of Responsibility:
Case manage 12-15 families with acquiring housing and becoming self-sufficient; confirm eligibility; Secure medical, psychological, & other factors contributing to situation; Identify affordable housing, assist with moving process; Case management for up to 6 months with each family moving toward self-sufficiency; School-aged children regularly in school; participating families have access to health & dental care; individual, family and group counseling.
Additional Areas of Responsibility:
Help clients modify behavior patterns; increase understanding of self, personal challenges, responsibilities; Refer clients to appropriate resources/organizations; keep accurate records & case files; visit clients in their homes; Engage service provider, business and faith community partners; Track outcomes; Provide supervision for small staff; Work with grants dept. to apply for funding opportunities & help prepare annual budget.
