Main Areas of Responsibility:
The Pathways Navigator connects clients in the asylum seekers or housing/homelessness programs with appropriate services to improve their health outcomes, overall wellness and self-sufficiency. The position guides clients through designated “pathways”, using the “Pathways to a Healthy Bernalillo County” program, to mark their progress towards improved health and wellbeing.
Additional Areas of Responsibility:
-Assess eligibility and level of readiness for adults/families to enroll in Pathways
-Provide basic family/individual facilitation to develop pathways
-Coordinate and follow up on referrals to other organizations
-Improve access to services for clients
-Provide appropriate general health and wellness education and self-advocacy skills
-Maintain client-tracking system
-Identify and document systems barriers
