Main Areas of Responsibility:
Nonprofit serving at risk, homeless youth and young adults seeking experienced person(s) to provide services to adjudicated and at-risk youth and youth adults ages 14-24. Will work individually with youth and their families, as well as link participants to services needed such as food, housing, medical and mental health services, academic counseling, life skills development, career guidance, case management, mentorships, internships, legal advocacy and follow-up services.
Additional Areas of Responsibility:
Collaborate with other team members to support youth in goal development through weekly check-ins and coordination of services to include life-skill and workforce skill development, as well as educational support such as tutoring, and/or linkage to support services in school, community, and home settings. Provide timely and thorough documentation and tracking for file compliance as well as educational/workforce and life skills group facilitation.
