Job Description
Main Areas of Responsibility: 

Deliver education, outreach and in-person assistance to link persons experiencing homelessness to appropriate health coverage.

Additional Areas of Responsibility: 

Must be willing to be certified both as a Health Care Guide and Determiner for PE-MOSA.

Other Information/Requirements: 

High School and 2 years in benefit enrollment or social services. Must have valid NM Driver's license and clean driving record. Computer proficient.
