Type of Job: 
Main Areas of Responsibility: 

Utilize evidenced-based best practices, including motivational interviewing, trauma informed care, and harm reduction when engaging individuals in their recovery
Meet individually with clients to assist with transition to services based on clients needs
Utilize valid triage and service prioritization tools and establish reliable pathways while providing expedited access to housing

Additional Areas of Responsibility: 

Work collaboratively with Case Managers
Coordinate open office hours and attends to client needs
Provide advocacy and linkages for clients
Cultivate and maintain relationships with landlords in Bernalillo County
Ensure the confidentiality of every client served by the program and Heading Home
Transition individuals into permanent supportive housing or proper placement based on the clients’ needs
Coordinate briefings and lease signings and attends as needed

Other Information/Requirements: 

Bachelor’s Degree in Social Work, Social Services or related field. In the absence of a pertinent degree, individuals with commensurate work experience related to the position may be considered for the position. Three years of experience in case management and strong administrative skills are a must with a minimum of two years of experience in managing programs that serve individuals experiencing homelessness or other low-income populations.
