Main Areas of Responsibility:
Utilize evidenced-based best practices, including motivational interviewing, trauma informed care, and harm reduction when engaging individuals in their recovery.
Meet individually with clients to assist with transition to services based on clients’ needs.
Utilize valid triage and service prioritization tools and establish reliable pathways while providing expedited access to housing.
Work collaboratively with Case Managers.
Coordinate open office hours and attends to client needs.
Additional Areas of Responsibility:
Identify qualified individuals from the Coordinated Assessment list and complete all required documentation with guests
Coordinate briefings and lease signings and attends as needed
Coordinate annual housing inspections with the Housing Inspector
Conduct annual updates for Rental Assistance Clients
Assist in the facilitation of the monthly Care Committee Meetings
Complete VI-SPDAT surveys as needed
Participate in weekly supervision to review cases and issues related to level of service
