Main Areas of Responsibility:
Under the direction of the Director of Patient Support Program, the Medical Case Manager works independently to provide long term coordination of care and services to patients in order to assist them to overcome the problems and needs associated with medical conditions, including addressing barriers to healthcare access.
Additional Areas of Responsibility:
Provides in-depth case management services to medical patients, with special attention and understanding of the needs and circumstances related to barriers to health care and the psychosocial support needed to cope with chronic, acute or terminal illness. Carries out Case Management within the context of on-going needs, care and/or treatment planning, and monitoring.
