Intensive Case Manager

Main Areas of Responsibility: 

Casa de Salud is excited to partner with the UNM Department of Family Medicine, under a contract with the City of Albuquerque, and is looking to hire a full-time Intensive Case Manager who can assist individuals with co-occurring conditions including alcohol/substance abuse.

Additional Areas of Responsibility: 

The Intensive Case Manager provides education, care coordination, and access to community resources. The Intensive Case Manager would work in both clinic and community settings, including with clients at their homes. Goal is to improve quality of life and maintain the highest possible ability to function and build self-efficacy, within the community.

Other Information/Requirements: 

Ideal candidate possesses:

- Relevant experience
- Strong interpersonal and communication skills
- Ability to work effectively with a wide range of constituencies
- Desire to work in an integrative clinic
- Ability to work in a trauma-informed care, social justice-informed care, culturally humble, anti-racist framework
- Strong documentation skills and ability to maintain records
