Main Areas of Responsibility:
The Intensive Case Manager (ICM) provides services to identified cases, assisting with stabilization, promoting self-sufficiency and long-term integration for clients and/or family members to special populations including single mothers, elderly refugees without family support systems, unaccompanied minors and other refugee populations with intense medical, mental, health, or social integration issues.
Additional Areas of Responsibility:
• Ensures intakes and assessments are provided in compliance with ICM protocols; including providing a mental health screen using the RH-15 tool, training staff and interpreters as necessary.
• Initiates service plans for all ICM eligible clients, reviews and closing cases in a timely manner and in compliance with ICM protocols.
• Provide solution-focused counseling and crisis intervention support as needed to clients.
