Main Areas of Responsibility:
• Support Housing Placement including housing referrals, housing search, housing application process, and other move-in activities
• Create a Housing Sustainability Plan with each shelter resident including:
-Budgeting
-Life Skills Training
-Direct/Assist in applying/securing all eligible entitlements
-Reducing other barriers to housing
• Conduct interim forms as needed
• Monitor resident progress through office visits and telephone contact
Additional Areas of Responsibility:
• Provide information and referrals to community resources including but not limited to:
-Health Care
-Behavioral Health Services
-Employment Development/Employment Search
-Daycare
• Maintain case notes, appropriate records, chart documentation and other information as required
• Attend weekly case management meetings and give case updates
• Advise Director of Programs of adverse changes in resident progress or needs for support
• Conduct intakes, discharges, and coordinated assessment as needed
