Main Areas of Responsibility:
The Housing Program Case Manager will screen participants; support housing placement; negotiate leases; create a Housing Sustainability Plan focused on housing stability (budgeting, life skills); assist in applying and securing entitlements; conduct assessments and monitor client progress in person and by telephone; provide crisis intervention if needed; assist with landlord/tenant relationship; and provide referrals to Health Care, Behavioral Health Services, Employment, and Daycare.
Additional Areas of Responsibility:
The Manager will maintain case notes, records, and documentation as required by funding sources; attend meetings and trainings; advise Director of Programs of changes in participant progress or needs for support; work with Director of Programs on program projections, management, and development; provide notice of program completion to landlord/program participant; conduct discharges; know emergency procedures; provide On Call Shelter Backup; and other duties as assigned.
