Housing Case Manager

Job Description
Type of Job: 
Full-Time
Main Areas of Responsibility: 

Operates housing first based program and homelessness prevention programs; assists individuals in obtaining and maintaining stable housing; ensures that programs are compliant with all contract requirements; and complies and maintains records and program data.

Additional Areas of Responsibility: 

Maintains on-going communication with clients; and provides wrap-around support and referral to resources.

Other Information/Requirements: 

Strong administrative skills, including organization and attention to detail; and previous work experience with individuals experiencing homelessness is preferred.
