Main Areas of Responsibility: 

Community Support Worker (CSW) needed for Heading Home project at St. Martin's Hospitality Center. CSW will seek to identify homeless clients with high scoring index of physical and mental illness to be placed in housing. Will perform CCSS services for clients, including notes.

Additional Areas of Responsibility: 

Ability to interact with homeless/SMI population

Other Information/Requirements: 

Must have at least 1 year experience working with homeless/SDMI population.
BA in related field required.
Must have reliable vehicle and valid NM license.
