Main Areas of Responsibility:
•Support client in locating an apartment &ensure appropriate inspections are conducted
•Negotiate & obtain leases according to grant guidelines &agency procedure
•Create an Exit Plan that is focused on housing stability with each program participant
•Monitor client progress through home visits and telephone contact
•Provide information and referrals to community resources
Additional Areas of Responsibility:
•Maintain appropriate records, and other documentation, as required by funding sources
•Attend weekly case management meetings and give case updates
•Advise Housing Director of adverse changes in client progress or needs for support
•Perform other duties as assigned
