Family Service Coordinator

Job Description
Main Areas of Responsibility: 

Seeking FT Family Service Coordinator to assist and enable families of children from age birth to 3, to access and if determined eligible, receive early intervention services. Coordinate intakes of clients, evaluations, assessments and determining eligibility. Facilitate and participate in the development of the initial and annual Individualized Family Service Plan (IFSP).

Additional Areas of Responsibility: 

. Assists family in receiving all identified services, coordinates IFSP services, refer/link families with community support/services as needed.

Other Information/Requirements: 

BA in early childhood education, social work, or related field required. Position located in Espanola. Must have reliable transportation. Bilingual a plus.
