Main Areas of Responsibility:
Provides developmentally based early intervention &/or early prevention services, service coordination, & case management support to women & families during pregnancy & through the third year or fifth year of a child’s life. The EIS functions as part of a multidisciplinary &/or interdisciplinary team & participates in standardized evaluations &/or ongoing assessment of children’s developmental, social & emotional, & family functioning in collaboration with other team disciplines.
Additional Areas of Responsibility:
The EIS participates in child & family medical visits & supports access to medical care & other needed services. Also provides linkages to other critical community resources and is knowledgeable about those available in the community served. This individual works with high risk populations that include, but are not limited to families &children impacted by substance abuse, caregiver mental health concerns, poverty , &other medical conditions placing children at risk for poor developmental and health outcomes.
