Main Areas of Responsibility: 

Role summary: assisting families through intake and eligibility determination process, and facilitating the IFPS process through transition. Provides information about, and makes referrals to, other community resources; coordinates and ensures the delivery of all services. Provides services that include consultation with the family.

Additional Areas of Responsibility: 

Principal accountabilities: Performs duties of Service Coordination and Developmental Specialist as defined by the NM Department of Health. Family Infant Toddler Program for Children referred to the early intervention program at LifeROOTS. Provides interim service coordination and completion of family and medical history, arrangement for evaluation, development of IFSP, coordination of IFSP services both within and outside the agency, and assistance to access services and transition planning and assistance.

Other Information/Requirements: 

Essential skills and experience:
1. BA/BS degree in Social Work, Early Childhood Education, Counseling or related field
2. Must maintain Developmental Specialist Certification in accordance with FIT Program regulations (7.30.8 NMAC and DDSD policy
3. Knowledge and understanding of Early Intervention and/or NM FIT Program
4. Must complete required training
