Job Description
Main Areas of Responsibility: 

Dismas House, a program of St. Martin's Hospitality Center, has an immediate opening for a Community Support Worker to provide CCSS services. Dismas House is a program for adults transitioning from jail or prison into society. The program is dedicated to serving formerly incarcerated adults. This position will provide case work services including intakes for their own clients.

Additional Areas of Responsibility: 

Assessments
Treatment plans
Discharge planning.
Assist and advocate for clients with short term needs/supports and long-range planning for intergration back into society.

Other Information/Requirements: 

BA in human service or corrections related field desired.
AA with 1-3 years experience will be considered.
HS diploma with 5 years experience with this population may be considered.
