Coordinated Entry System Navigator

Main Areas of Responsibility: 

Coordinated Entry System Navigator is responsible for working with households experiencing homelessness, building an early rapport, and supporting people to the point where they are able to secure permanent housing. Duties include: Administering the common assessment tool and triage households; Assisting households with accessing permanent housing opportunities including Permanent Supportive Housing, Rapid Re-Housing, other subsidized housing, and affordable housing.

Additional Areas of Responsibility: 

Input and review data in the Homeless Management Information System (HMIS) for accuracy and completeness; Coordinate with community partners from various agencies and programs that utilize the Coordinated Entry System; Possess an understanding of and practice cultural sensitivity through open dialogue and self-exploration with diverse groups, while providing direct services; Performs other incidental and related duties as required and assigned.

Other Information/Requirements: 

Two years of direct service experience in human services; High school diploma required; College degree a plus; Reliable transportation and valid NM driver's license: Ability to work flexible hours, including some weekends and evenings; Ability to work independently, as well as, an effective and collaborative member of a team: Knowledge in data entry and database reporting.
