Community Support Worker - Part Time

Job Description
Main Areas of Responsibility: 

Provide CCSS Comprehensive Community Support Services as a Community Support Worker to recently homeless individuals living in a residential recovery program on a part-time (20 hours per week) basis.
Provide resources and referrals, complete assessments, develop treatment plans, prepare case notes and transport clients if needed.

Additional Areas of Responsibility: 

Days would be Monday, Tuesday, and Thursday. 2 days working 8 hours and one day working 4 hours.

Other Information/Requirements: 

BA or BS in human service related field required
Must have at least 1 year experience working with SMI, Substance Use Disorder and/or homeless population.
Must have reliable vehicle and good driving record.
